Reinstate a Canceled Policy
Reinstate a Canceled Policy
Follow the steps below to request that a subscriber’s terminated policy be made active again.
 
Step 1:   Look up the subscriber’s canceled policy using View Eligibility
 
Step 2:   Click the “Reinstate Terminated Policy”
Note:
A policy can only be reinstated within 30 days of termination, so if there is no hyperlink, you will
need to re-enroll the member in a New Policy.

Step 3:   Complete the contact section of the electronic form and click “Submit”
Our Membership & Billing staff will process your request within 3-5 business days.
