Annual Group Information Form Reminder
Beginning with April renewals, Excellus BlueCross Blue Shield will start mailing the AGIF requests to community rated groups quarterly. Groups renewing benefits in April, May or June, will receive the invitation either through e-mail or paper mail. An AGIF completed in 2016 will need to be on file.
We appreciate your taking the time to ensure your groups complete this important information as soon as possible prior to their renewal. It is important to have this information accurate as your group’s health insurance benefits and premiums may be affected by information on this form.
The interactive form can also be accessed at any time by visiting www.excellusbcbs.com/verifygroupinfo. Once the questionnaire is completed and signed by the group, the group should save the form and submit it to AnnualGroupInformationExcellus@excellus.com.
