Accessing the Web Version of the Annual Group Information Form
To locate the web version of the Annual Group Information Form (AGIF) follow these:
- Log in to your account at ExcellusBCBS.com.
- Once logged in, you can find the form in the Broker page or the Employer page.
- From the top navigation bar, select “Enroll & Update”
- Click on the “Group Information Form” link found on the left hand column
- Once the form is opened, enter the group number to start completing the form.

If the group does not have a login, they can request access at our registration page.
IMPORTANT: When a group is completing the web registration form, please be sure to use this link ONLY. Using any other link may impact your access for that group.
