Underwriting Process Updates for Group Information
Due to various state and federal regulations, Excellus BlueCross BlueShield is required to obtain certain information regarding groups. As a result, we have developed two forms for groups to complete. These forms are available on Blue on Demand.
In July, an informative notice will be sent out explaining the regulations and how they vary based on the type and level of information needed. This notice also helps to outline why the information is important.
Note: Groups that only have dental with us will not be included in the mailing. For experience-rated dental groups, the Annual Group Information Form (AGIF) will be required prior to the release of the renewal. For community-rated groups, benefit changes will not be processed unless the Annual Group Information Form is on file.
Annual Group Information Form Notification
- The AGIF notification will be emailed directly to insured groups for which we have a current email address on file. Instructions will be included to complete the interactive form and submit through the Web.
- The AGIF paper notification will be mailed directly to insured groups for which we do not have an email address on file. Instructions will be included to complete the interactive form and submit through the Web.
- The notification will include a phone number to request a paper form for any group who does not want to complete and submit the form through the Web.
- The forms must be completed and signed by the authorized group representative. The broker cannot complete and sign the form on the group’s behalf.
New Business Group Information Form
The New Business Group Information Form has been revised to include updated information. For small groups, we have also updated the Small Group Underwriting Packet.
For all insured groups with effective dates beginning September 1, 2013, we require the revised New Business Group Information Form. Please be sure to replace any old forms you have on file with this new version.
