Health and Recovery Plan (HARP)

Some of the benefits to support your health:

• Mental health care that includes inpatient and outpatient treatment, partial hospitalization care, individual and group counseling, continuing day treatment, and crisis intervention services.
• Substance use disorder services to include outpatient and inpatient treatment, residential substance use disorder treatment, opioid treatment, and detox services
• Home and community services to help you live independently, find housing or a job, return to school and prevent crises.

You may be eligible for HARP with the following criteria if you:

• Are 21 or older
• Qualify for Medicaid
• Do not have Medicare
• Have received a letter from New York State confirming your eligibility to enroll in HARP
• Live in our service area (counties include Broome, Herkimer, Livingston, Monroe, Oneida, Onondaga, Ontario, Otsego, Seneca, Wayne and Yates)
• Live in Erie or Orleans County

Caring services to help with basic needs include:

• Family planning
• Help with housing, food, transportation and more with Social Care Network partners
• Consumer-directed personal assistance services
• Transitional Age Youth (TAY) resources for youth transitioning to adulthood
