You can apply for your child's coverage online through the New York State of Health by visiting www.nysofhealth.ny.gov. For help applying, call NYSOH's Consumer Service Center at 1-855-355-5777.
Please note, your child's application must be processed by the 15th of the month in order for coverage to begin on the first day of the next month. If the application is processed after that date, coverage will be effective the first day of the next month.
Please note, if you are a new applicant, your coverage will be effective on the first day of the month that your application is received. If you had coverage through the NYSOH under a different program or plan and are switching to the Essential Plan, your coverage will begin on the first of the month following your plan selection.
